There are a number of dramatic contrasts between Pediatric and Adult medicine that often complicate transition process. For example, the dynamics of an adult medical visit are completely different than in a Pediatric medical visit. Also, coordination of care between medical providers is often more complicated and is more patient driven. Participants of this webinar will be presented with strategies to manage these and other challenges inherent to the transition process.
